Regenerative Endodontics: Current Status and Future Research Strategy

Abstract:

Regenerative endodontics is a biologically-based procedure designed to replace damaged tooth structures, including dentin pulp tissue. Regenerative endodontics applies all the principles of regenerative medicine, utilizes specific cells, three dimensional scaffolds growth factors alone or in combination to regenerate new tissues. Regenerative endodontic procedure has been used in clinics to treat necrotic permanent teeth that having immature root formation with open apex.

In this lecture, I will first talk about the biological basis of the regenerative endodontic clinical procedures from the tissue engineering aspects. We will discuss the goals of current treatment: the primary goal of regenerative endodontic treatment is to promote continued formation of root apical closure; the ultimate goal is to regenerate a functional pulp-dentin complex. Although the primary goal has been accomplished in some cases, the ultimate goal of the regenerative endodontics has not been achieved.

Next, we will discuss the clinic case selection the current standard clinical protocol established by the American Association of Endodontists (AAE). The details of the protocol its scientific evidence will be discussed. Then we will reviewed the outcome of current regenerative endodontic treatment using the clinic, radiographic histologic criteria, compared them with the alternative treatment modalities such as the apexification.

Finally, I will talk about the regenerative endodontic by tissue engineering method. I will describe our recently studies in my research lab where we developed a novel biomimetic scaffold for the regenerative endodontics, other relevant studies on the growth factor release from dentin matrix.

As a summary, the aim of this presentation is to deliver a comprehensive introduction about the regenerative endodontics, including the biological basis the goals of treatment, the case selection current clinical protocols, as well as the outcomes assessment, also to give a glimpse of potential research strategies in the future.
